Sovereign:
Download and complete the claim form have your GP compete the medical section and send to Tobin Insurance Services for on sending to Sovereign or fax directly to Sovereign claim re approval on 0800-500-195. Write your policy number on each page. Please use a Black pen to complete the claim form. Make sure you have your specialist’s letter and costings for your claim.

ING:
Download and complete the claim form. Refer ING claim form instructions. Then send to Tobin Insurance Services for on sending to ING or fax directly to ING pre approval on 0508-464-666. Write your policy number on each page. Please use a Black pen to complete the claim form. Make sure you have your specialist’s letter and costings for your claim.

In all cases we strongly recommend that your seek pre approval for any medical insurance claim. Try and give as much time as possible for the processing of your claim. Sometimes the insurer has to go back to your doctor for further information and this all takes time. We recommend that you try and give the insurer 5 working days for pre approval claim processing.
